What is Phase IV Cardiac Rehabilitation?

Phase IV Cardiac Rehabilitation is a community based rehabilitation service that continues on from the Phase III Hospital services. A Cardiac Rehabilitation Instructor who has been trained by the British Association for Cardiac Rehabilitation runs the programme. The session format is similar to that of the hospital based Phase III, with a chance to socialise afterwards. On occasions lifestyle health and information sessions will also be provided.

Cardiac Rehabilitation - Can anyone Participate?

To attend the programme you require a formal referral form from your hospital Cardiac Nurses or via your GP’s CHD Referral Pathway.

What is involved in each Session?
Prior to each session everyone’s blood pressure and heart rate will be recorded. New comers will be welcomed to the centre and introduced to the programme. The hour session consists of a 15 minute warm up, followed by 30 minutes of a range of different exercises, most of which will be similar to those from the Phase III hospital programme. To finish there will be a 10 minute cool down and stretch. As you progress through the programme, exercise levels will increase to promote an increase in fitness. There is opportunity to ask questions and socialise with other participants after the session. This class have been established over five years and is a very friendly environment, you will not feel intimidated.
